My son started potty training at 14 months and used a sticker chart and loved it. 20 months is almost 2 years old so unless hes developmental delayed then I would try the stickers and after a week he gets a small treat
I think this is a good suggestion! If OP prefers to avoid screen time, we've had success playing a favorite song from Spotify when my daughter has to sit through tasks she dislikes (e.g. nail trims).
I’ve seen some people use inhaler time as screen time time. So getting to do a fun show during the duration of the experience.

As others have said, sticker chart is the way to go. If he gets all his stickers for x days in a ro he gets a reward. Make putting the sticker on part of the process as well as they usually love that. For this age I recommend a big print out for the week with. Picture of the inhaler on it. You can let him pick the stickers if he’s into it (my niece was way into dog stickers lol) or just use standard stars. To make your life easier I would create it for the week on A4 paper and then print a bunch. Have two on the wall at a time (this week, next week) so if he does muck up theres always lots of blank days ahead to make it back up.
At 18 months, a sticker chart working towards a reward isn’t a reward. He needs something he likes immediately during or after doing it. If he likes stickers, that can be a reward! Otherwise, a chocolate chip or small desired food, a short video, or doing a favorite activity immediately after are all good choices!
